What should shape a CEVNI meeting decision before two vessels enter a narrow fairway?

Short answer

Classify the meeting, read the signs and scope, and prove the full fairway is wide enough before either vessel commits

Article 6.03 requires meeting space to be unquestionably wide enough, while narrow-fairway, small-craft and local controls decide how that standard is applied. The first signal or vessel size alone cannot allocate unsafe water. Apply the classification before entry and revisit Classify the Traffic Situation Before Acting.
