Record What Matters

Professional records support safety, learning, legal compliance, insurance, and accountability. The level of detail should match the seriousness of the event, but significant incidents and near misses should not vanish into memory.

Record facts: time, place, people, weather, vessel condition, what happened, injury or damage, immediate action, notifications, witness details where appropriate, and follow-up.
